Our goal is to serve the behavioral health population, a group that has been historically undeserved, non-compliant with medications, and frequently hospitalized.

CONSIDERATIONS
We work with the behavioral health population, as well as with patients who have recently been discharged from the hospital, patients with disabilities, and patients with chronic illness. Our services include short-term and long-term plans, which we can adjust based on the changing needs of your family member. Please reach out to us to find out how we can help you.

A patient may be referred to Meridian by their physician, caregiver, family member, friend, hospital or nursing home, as well as self-referred.

We accept most MassHealth plans, including members on MassHealth waiver programs, as well as other insurances.
